Description of the contract

The purpose of the service is to manage patients who have developed, or are suspected of having, age-related macular degeneration (AMD). The aim and objective of this service is to provide timely access to quality assured assessment of visual acuity, retinal angiography and optical coherence tomography in patients suspected of having AMD within 2 weeks of referral. The service will deliver assessment and treatment for wet-AMD through anti-VEGF injection during a single visit, and continued follow-up and treatment until the disease has been stable for 6 months. The service will be delivered in appropriately equipped clinics with aseptic facilities and will be Consultant led (The Royal College of Ophthalmologists, 2009a; The Royal College of Ophthalmologists, 2013) It will do so by providing a high quality eye service founded on the principles of good practice and clinical governance. The service will seek to improve the patient’s experience by: ? providing a local service for patients, reducing journey times; ? improving access and choice and reducing waiting times; ? working closely with GPs, optometrists and secondary or specialist care colleagues to provide a seamless service and continuity of care; ? facilitating the development of ‘one stop’ patient consultations combining assessment, treatment, self-care instruction and discharge where appropriate; The service objectives are: ? To provide a convenient, accessible service for patients closer to home ? Possible multiple delivery points from which patients can access care ? Provide patient choice, demonstrating value for money, for full assessment and treatment of wet-AMD. ? Support appropriate referrals to specialist secondary care ? Support appropriate signposting and support patients to access alternative care ? Provide a safe and high quality service that meets the service specification service and clinical outcomes ? Meet evidence based clinical outcomes ? Acquire positive patient and carer/family feedback demonstrating action taken to improve the service as required
