Applications are invited from Voluntary and Community sector organisations interested in employing Community Practice Workers as part of the Prevention Matters development programme. Funding will be given for a period of 3 years.
The recruitment of Community Practice Workers is an outcome of a series of workshops and multi-agency meetings held earlier in the year to co-design a prevention model for Buckinghamshire. Community Practice Workers will be aligned to GP localities throughout Buckinghamshire, working closely with primary care teams. We are looking to introduce up to 14 (full time equivalent) posts from October 2012. The focus of the role will be to support individuals who fall somewhere between the ‘eligible’ and ‘ineligible’ threshold for adult social care services – adults who may be experiencing difficulty in maintaining their independence and who, as a result, are expected to need more intensive health or social care support in the near future. It is anticipated that a significant number of referrals will be older people
Organisations are asked to nominate suitable individuals by returning the Registration Form by Friday 7 September 2012.
