Description of the goods or services required

Aneurin Bevan University Health Board are seeking suitable providers in respect of hospice at home service.

The proposed service will contritbute to an established comprehensive Hospice at Home service to support patients at End of Life. The service will be provided across Gwent on a 24/7 basis, 365 days a year. The aim is to provide an equitable service to all patients who may require the following support.

*Practical and social support

*Respite for Carers

Response will follow an assessment by a Clinical Nurse Specialist from the teamof the proposed service and may be a planned response or following a crisis.

The proposed service will contribute to an established comprehensive Hospice at Home service to support patients at End of Life.
