Leicester City Council are seeking a training provide to deliver the following training:
Event Title: How to Complete a Successful Hospital Discharge
Target Group: Team Leaders, Social Workers and Care Management Officers across the Adult Social Care and Safeguarding Division.
Number of staff to be trained per event: 20
Number and length of events: 10 events, 1 day each
Dates if known: 1-2 before April and the remainder by October
Venue: Leicester City Council will provide a venue
In October 2012, the division underwent a large organisational review. One of the implications of this was that the 3 specialist hospital social work teams were disbanded. All social workers and care management officers moved to a generic working model, meaning that workers without a background in hospital work could be asked to support a hospital discharge at any time. This has led to concern within teams that the process is not being correctly followed, as well as increase in charges incurred under the delayed discharge act. We therefore wish to roll out a series of one – day training events to address these issues.
To enable all relevant staff within social work teams to support customers to effectively plan for safe hospital discharge.
• To engage in learning and activities which enable understanding of the Delayed Discharge Act and its implications.
• To receive information about Leicester City council’s agreed protocol’s for the implication of this act.
• To explore examples of how to complete a Decision Support Tool, and to practice this in an exercise.
• To hear from Health and Social Care Managers about their expectations, and how we can best work together.
• To increase participants’ ability and confidence in facilitating successful hospital discharges in a joined up and timely manner
• To Ensure Leicester City Council does not incur unnecessary charges under the Delayed Discharge Act.
