Within NHSScotland, the Knowledge Network (www.knowledge.scot.nhs.uk) has been designed as the key vehicle to implement the successive knowledge management strategies developed by NHS Education for Scotland (NES) since 2003, all based consistently on the principle of benefitting health and care by enabling application of knowledge at point of need. It is underpinned by NES’s support for development of workforce skills in finding, sharing and using knowledge, including developing the role of librarians and other knowledge brokers. Map the current state of knowledge support (technology, people/skills, processes) across the local authority, NHS, voluntary and private sector partners in three Change Fund / Health and Social Care Partnerships. Collectively with stakeholders from all sectors, define the “ideal” future state of knowledge support. This should reflect the vision, principles and approaches set out in national knowledge management strategy for health and social care. It should also contribute clearly to delivery of Quality Outcomes in line with the 20:20 vision for the future of integrated, continuous care and support, enabling people to live well at home or in the community. Design an improvement plan comprising practical and affordable improvements in knowledge support, to bridge the gap between current and future state.
