The London Borough of Merton (LBM) has a requirement for an experienced and qualified organisation(s) to design and deliver a primary care focussed NHS Health Checks programmes that maximises the reach and uptake of NHS Health Checks in key priority groups of the eligible GP registered population of Merton.
As part of the Merton prevention framework and linked to the recently commissioned Healthy Lifestyle Service, the provider will be expected to maximise the reach and uptake of NHS Health Checks in priority groups.
