II.1.5)Short description of the contract or purchase(s)
The London Boroughs of Hounslow and Ealing and Hounslow’s and Ealing’s CCG’s are looking to develop a new Service, which will meet the requirements of an integrated health and social care delivery model and increase flexibility and choice in the market.
Rather than a traditional home care Service, we seek to commission an outcomes-based Service across health and social care, which demonstrates a commitment to better quality, dignity and safety with the core aim of enabling adults, children and young people to remain in their own homes, living independently for as long as possible.
The parties intends to let a Framework Agreement for the delivery of high-quality Personal Care Services on behalf of Hounslow Clinical Commissioning Group, Ealing Council and Ealing Clinical Commissioning Group. This Framework will effectively be a list of approved Providers that have demonstrated that they have the appropriate skills and experience to meet the care and support needs of adults, children and young people. Providers included on this Framework will not be guaranteed a minimum volume of hours. Hounslow Council, Hounslow Clinical Commissioning Group, Ealing Council and Ealing Clinical Commissioning Group may enter into call-off contracts with Providers on this Framework.
The London Boroughs of Hounslow and Ealing and Hounslow and Ealing CCGs are committed to whole system working and improving Service User experiences and outcomes through properly coordinated health and social care Services. Our shared vision is to enable joint working between public and private organisations across the health and social care system to give people the best possible chance to remain as independent as possible for as long as possible in the community. We aim to bring together professional expertise and collective wisdom so that Service Users and carers can be partners in their own support, and where possible take responsibility for directing their own care.
The Service will be provided in all of the boroughs’ localities. Providers may choose to operate across all of the London Boroughs of Hounslow and Ealing, or alternatively within certain geographic areas within the boroughs. Service provision on a locality basis aims to promote and support the involvement of local community and voluntary organisations as well as small specialist Providers.
Providers on the Personal Care Framework will become an integral part of local communities in both boroughs, working in partnership with GPs and community health and social care staff and the high quality care and support they will deliver will enable us to achieve our vision of supporting as many people as possible in their own homes.
