Description of the contract

At the heart of a PHB is a support or care plan developed in partnership between a care planner and the individual. A care plan is a record of the discussions and agreements between the person and their health team including their health needs, the outcomes they want to achieve, how they intend to use their budget and how they will manage the plan. Many people will have clear views about how they want to use their PHB and will be able to plan without support or with the support of family and friends, however, others will need support. While clinicians need to be closely involved in support planning, pilots have generally found that individuals are better supported by an independent person, such as a support planner or broker. These independent advisors can help individuals identify their goals, research different ways of meeting those goals and ensure that clinical needs are adequately met. This is particularly important in continuing healthcare where individuals often have significant and complex needs. Support planners or brokers can also ensure that clinicians are brought into the support planning process from the beginning to discuss with individuals how their clinical needs can be best met in line with their wider support plan. NHS Surrey Downs, Surrey Heath, North West Surrey, East Surrey, Guildford and Waverley and North East Hampshire and Farnham CCGs have recognised the importance of support planning and brokerage services to support the effective implementation of Personal Health Budgets locally. The Advisory Service will assist budget holders to choose services to meet their health outcomes and develop a plan to describe these.
