Lewisham is seeking a Provider who will work creatively and in partnership with the Council and Phoenix Community Housing Association (Phoenix) to deliver an Extra Care service at Phoenix’s Hazelhurst Court scheme. Hazelhurst Court is the most recent in a programme of Extra Care developments in the borough. Hazelhurst Court comprises of 58 x 1 bed and 2×2 bed flats across two blocks.
The service will include, but is not limited to:
— Provision of personal care and support, including medication.
— Support to maintain independent living skills.
— Support to develop social relationships.
— On site 1st line emergency response to tenants.
— Co-production of the ‘Peace of Mind’ service.
