II.1.5)Short description of the contract or purchase(s)

The City of London will be procuring an integrated NHS Health Checks, lifestyle weight management and physical activity service.
1. Overview of the Service.
1.1. The purpose of the service is to replace the services which are currently provided by four external providers covering NHS Health Checks, Exercise on Referral, Dietetics and Lifestyle Weight Management, and Community Health Engagement.
1.2. The new service will bring together NHS health checks, obesity and physical activity services under an integrated system;
1.3. We intend to commission this under a lead provider model, allowing specialist providers to work together utilizing their skills and expertise in each of the different areas. The service model therefore can be described as having 6 components. The components are designed to link and work together and it is the responsibility of the Lead Provider to make this happen.
The service components include:
— Service Component 1: lead Provider Management Function. Coordinating and performance managing the other components.
— Service Component 2: triage/Single Point of Referral: Ensure assessment by a health care professional prior to entering service components 4 or 5.
To assess readiness to start a structured programme and set initial client centred goals, directing the client to the most appropriate service according to the care and treatment pathway. This will include liaising with and referral in to the wider City and Hackney joint Public Health and Clinical Commissioning Group obesity services pathway, which will include higher tiers of service. This will be available to City residents and low paid workers. For those clients not suitable for other services, signposting to relevant universal community activities and services.
— Service Component 3: NHS Health Checks (Primary Care and Community). Provision an NHS Health Checks Project which will carry out health checks in a variety of community settings across the City of London. This will be available to City residents and low paid workers. Provide appropriate referrals in to Service Component 2. The service will work with The Neaman Practice in the City who will provide health checks to their registered patients. This will include having relevant data sharing arrangements in place.
— Service Component 4: exercise on Referral. The provision of a structured exercise programme, with follow up after 6 months and 12 months. The service will advise, support and motivate participants who would benefit from increased physical activity. This will be available to City residents and low paid workers. Depending on the needs of the individual, this should be designed in conjunction with Service Component 5. Clients entering this service must have had assessment under Service Component 2.
— Service Component 5: dietetics and Lifestyle Weight Management. The provision of a structured multi-component weight management programme to support clients to develop healthy eating habits, increase their levels of physical activity and to learn from behaviour change strategies — with follow-up at 6 and 12 months. This will be available to City residents and low paid workers. Clients entering this service must have had assessment under Service Component 2.
— Service Component 6: community Health Engagement. Delivery of a variety of weekly activities and/or information sessions to promote health and well-being among residents of diverse communities in the City. This will be available to all City residents. Increase resident participation in activities to address identified health and well-being needs, including signposting to relevant universal community activities and services. This function will be involved in multi-agency working to help highlight the particular needs of the community.

Joint and several liability. Should the successful tenderer be a consortium of companies, the lead partner will be responsible for completing the commission on behalf of the consortium and all payments will be made to the lead partner for services rendered by the consortium. Support partners will be required to complete a collateral warranty agreement in favour of the Contracting Authority.
