This Provider Panel is being created without any restrictions placed on the number of successful Providers who may be appointed as Panel Members.
Provider Panel to be periodically re-opened for the membership to be refreshed (initial opening after 6 months of operation, then on annual basis).

Contract Period is 1.10.2016 to 31.3.2018, then 2 further potential extensions of 12 months (from 1.4.2018 and 1.4.2019).
