Provision of Short Breaks and Other Support Services

Description of the contract

Provision of Short Breaks & Other Support Services which will be split into the following Lots:- Lot 1 – Early Years & Support Services for Families of Disabled Children: Birth to 18 Years. Home-based learning, term-time play activities for under 5’s, specialist key worker service, disabled parent forum, holiday activities for siblings. This service will be let as a block contract for 3.5 years with the option for two 1-year extensions subject to service need and performance. Lot 2 – Short Breaks for 9-18 Year Olds Range of after school, weekend and holiday activities/clubs, overnight & weekend fund camps, Saturday youth club and advocacy service for disabled children/young people. This service will be let as a block contract for 3.5 years with the option for two 1-year extensions subject to service need and performance. Lot 3 – Short Breaks for 16-18 Years: Transition Support Targeted transition support through a buddying service and independent living project. This service will be let as a block contract for 3.5 years with the option for two 1-year extensions subject to service need and performance.
