Tender for the Provision of Pathways to Independence (Accommodation & Support for Vulnerable People aged 16-25 years) and Socially Excluded Services in Somerset – REF P2I /P4SE 2012

Pathway to Independence – REF P2I

Somerset County Council, in collaboration with the 5 District and Borough Councils, is seeking to commission Accommodation and Support Services (elements of which are statutory) that will meet the needs of vulnerable young people aged 16 to 25 years old, including:

– Homeless 16/17 year olds

– Vulnerable Young People, including Care Leavers aged 16 – 25

– Young parents and expectant mothers aged 16 – 25

– Young Offenders

Based on evidence and the poor outcomes for young people who leave home at a young age in a crisis or unplanned way and become homeless, our primary focus is prevention, as long as it is safe for young people to remain at home/with extended family. The commissioned Services will enable young people to progress from support and supervision to independent living, what we in Somerset have called a ‘Pathway to Independence’ (known as P2I) and will provide good outcomes for young people

– Improved health and well being

– Economic well being

– Making a positive contribution

– Independent Living

– Being Safe

These will be achieved through:

– A service based on the prevention of homelessness

– A service based on assessed need not legal status

– A single Access Point or Gateway in each District that will provide advice and information on housing issues and which manages referrals to Young People’s Services

– Homelessness prevention for all ages, with a strong focus on 16/17 year olds

– Emergency Accommodation

– Supported Accommodation

– Move on Accommodation

– Young person centred Independence plans

Socially Excluded REF P4SE

Somerset County Council is seeking to commission Accommodation and Support Services that will meet the needs of vulnerable people including:

– Single Homeless or at risk of becoming homeless requiring emergency accommodation

– Single Homeless or at risk of becoming homeless requiring short term support

– Single people experiencing poor mental health

– Offenders, ex-offenders discharged from prison and those at risk of re-offending

Following a Needs Analysis which was conducted in November 2011, there is an identified requirement to develop a “whole system approach” to provide more inclusive “end to end” integrated services, which include prevention and resettlement to support vulnerable adults in Somerset who are socially excluded, with a focus to reduce revolving homelessness and provide good outcomes for vulnerable people

– Being Safe

These will be achieved through:

– A countywide Ex-Offender Service

– Direct Access Services, one East County, one West County

– Accommodation Hubs with Outreach and Resettlement for Single People with Assessed Support Needs

– Accommodation Hubs with Outreach and Resettlement for Single People experiencing poor Mental Health

– All services meeting the strategic objectives for the core needs of the client group

– All services being delivered in accordance with relevant legislation and best practice pertaining to the client group.

– All services working in partnership and positive co-operation with appropriate complementary agencies to provide an interface to secure personal solutions for each client.

The contract for the Services will be with Somerset County Council and will commence on April 1st 2013 until March 31st 2016 with the possibility to extend 2 X 12 months, dependant on Provider investment and service quality i.e. successful delivery of outcomes for young people and socially excluded.
