Description of the contract

NHS Bradford Districts CCG and NHS Bradford City CCG wish to contract with a provider to provide accessible information for patients registered with a Bradford GP practice. Locally the Accessible Information Service will provide a high quality service for Bradford City and Bradford Districts Commissioners and People with accessibility needs, which will include but not be limited to Protected Characteristics and specific formats, BSL- British Sign Language, Easy Read, Audio Recording e.g. CD’s DVD’s and MP3 plus local community languages. Bradford Commissioners are committed to the systematic provision of Accessible Information in community settings to identify people’s risk of specific conditions and ensure appropriate treatment and referral to medical and lifestyle services. Ultimately this service aims to improve the health of the local population and reduce health inequalities across Bradford City and Districts CCG areas. The service will require innovative methods in order to successfully target specific health needs and general population benefit. Objective 1: How to guide and principles that create a way of working where Accessible Information is a part of clinical pathways and effective communication, self-care and involvement by those who require this service; Objective 2: Design and Implement targeted health campaigns; Objective 3: Develop and Maintain a Library; Objective 4: Dissemination of materials The commissioners will require mobilisation of services as soon as possible after contract award.
