Funding provided in this Contract is for the provision of a minimum of 1.4 Whole Time Equivalent Third Sector Brokers, to be based within a multi disciplinary intermediate care service providing a seven day service across the Wrexham County Borough including the Wrexham Maelor Hospital and will be managed by Wrexham County Borough Council Adult Social Care department.

THIRD SECTOR BROKERAGE SERVICE INTERMEDIATE CARE FUND 2014-15

Wrexham County Borough Council is in receipt of Welsh Government Intermediate Care Funding and is looking for tender applications from organisations that are able to meet the required service aims and objectives as detailed below.

The Provider will work with individuals, professionals and other Third Sector organisations in Wrexham County Borough. Funding provided for the provision of 1.4 WTE Third Sector Brokers to be based within and part-managed by the Wrexham Intermediate Care Team (Adult Social Care).

The Third Sector Broker post is a new post within the Intermediate Care Service and forms part of a one year project to enhance the size and scope of the Service across Wrexham to support older people to remain as independent as possible within their own homes and communities, avoiding unnecessary admission to hospital and supporting timely discharge home where a hospital admission has been necessary.

The aims and objectives of the service are:

• To work with the Wrexham Intermediate Care Team to ensure that information on third sector provision is readily accessible to relevant professionals and to broker service provision in response to identified need.

• To raise awareness of third sector provision amongst members of the Intermediate Care Team

• To contribute to the work of the Intermediate Care Team as directed by the Adult Social Care, Intermediate Care Team Manager, and Head of Service for Older People, and to broker access to third sector services at regular multi-disciplinary team meetings.

• To implement procedures for signposting and referral to third sector organisations, underscored by sound information sharing protocols.

• To provide, and make available, appropriate information to and from the Association of Voluntary Organisations in Wrexham (AVOW) for the development and maintenance of a directory of third sector service providers for use by the Intermediate Care Team and other relevant professionals/ partners

• To work with third sector providers and service commissioners to ensure that adequate quality assurance/ clinical governance/ performance management systems are in place to underpin safe referral processes

• To maintain accurate records and produce regular reports in line with monitoring requirements of the Intermediate Care Team and the project funders.
