Lot 1 — Single Point of Access for the Warwickshire Fitter Futures Programme and Physical Activity/Healthy Lifestyle on Referral Programme.
The management and co-ordination of a single point of access for the Warwickshire Fitter Futures Programme; Lots 1, 2 and 3.
Warwickshire’s Exercise on Referral Programme has been commissioned by Public Health Warwickshire for approximately 12 years. The aim of the service is to increase physical activity levels in individuals with health conditions which meet a number of criteria including: weight management; diabetes; mental health illness; cardio vascular disease; early to mid-stage dementia; muscular/osteo skeletal conditions.
Individuals are referred to a 12 week supported programme of physical activity by a health professional. Individuals are encouraged to sustain activity post the 12 weeks. Year on year there is an increase in individuals being referred to, completing the programme and sustaining physical activity post programme.
Lot 2 — Structured Family Weight Management.
The aim of this service is to halt the rise in obesity in primary school age children using a whole family evidence based approach. Since January 2010, Rugby Borough Council and Nuneaton and Bedworth Leisure Trust have been commissioned by Public Health Warwickshire to deliver this service which is underpinned by a model of health behaviour change, in the Boroughs of Rugby, North Warwickshire and Nuneaton and Bedworth.
Lot 3 — Weight Management on Referral.
The service will deliver a weight management on referral service, referred by health professionals through a single point of access, to young people aged 12-16 identified as overweight/obese and to adults aged 16+ with a Body Mass Index >30 (or >28 with co-morbidities) across Warwickshire. The programme will work in conjunction with and support the provision of the Physical Activity/Healthy Lifestyle contract.

There are two components to the Warwickshire Fitter Futures service. The Provider will be required to deliver both components. These are as follows:
1. The Management and co-ordination of delivery of a Warwickshire wide service to improve health through increasing physical activity levels through multi component on referral programmes with a specific focus on young people aged 12-16 and adults aged 16+ who have already being diagnosed with a health condition. The Provider will be required to identify, recruit, co-ordinate and oversee physical activity/healthy lifestyle organisations to deliver the programmes to the target population.
2. The Management and co-ordination of delivery of a Warwickshire wide adult weight management on referral service to improve health though reducing overweight and obesity in young people aged 12-16 and adults aged 16+.
