Title

Care at Home and Support at Home or in the Community for Children and Young People with complex need

2.2

Description of the goods or services required

The purpose of this service is to provide care and support at home (and in the community) for children and young people who have a substantial and enduring disability or chronic health or life limiting condition. It will involve working in partnership with families. The Council currently commissions around 400 hours per week of care and support for children. As this is a framework there are no guaranteed hours and this is for new referrals only. The activities which will be provided as part of this service will include personal care and support with the activities of daily living as well as supporting families with complex needs to care for their children and young people in the family home and to access resources in the community. The main purpose is to enable children and young people who are assessed as requiring care and support to live as healthy, safe and independent lives as possible, enabling them to maximize their potential.
