St Helens Council is seeking to award a contract for the Provision of Carer Support Services within St Helens.
The service will be outcome focused and personalised to promote choice, control and independence for carers, tailoring solutions to carers’ specific needs as far as possible.
The service will support carers to recognise their needs and identify the outcomes they want to achieve in their life. Where appropriate the service will signpost carers to access statutory assessments and the appropriate health and social care services, to support meeting their desired outcomes. The service will also support the development of the local area to be truly integrated as well as building the capacity of other services and partner agencies.
The service will be flexible in its approach and work within the St Helens Cares accountable care model. This includes the identified “locality” boundaries, ensuring that resources are targeted to the where data and intelligence indicates need.
