The Trust are utilising the Competitive Dialogue Procedure to go to market for a Clinical Portal which will facilitate, manage and control the sharing of clinical patient information within County Durham & Darlington Foundation NHS Foundation Trust and between the Trust and other Care Providers. The solution will be required to share information held within Trust operated systems including those nationally provided; details of the systems with which the Clinical portal must interact will be set out in the Descriptive Document. The proposed Clinical Portal must provide a single, simplified view of patient data derived from existing systems; enable interaction with the data within the Portal, and enable the simple transfer of context into the underlying systems.
There is a requirement to provide an integrated care record across the whole Trust. The solution will enable the organisation to support integrated care, care closer to home, specialists spending time in both hospital and the community, better management of long term conditions with fewer hospital admissions and work closer with primary care, local authorities and other agencies.
In order to do this efficiently the Trust must provide a solution which will enable clinicians to access all the information they need to know about their patient at the point of care. The solution must provide a collaborative infrastructure for clinical workflows which traverses system and organisational boundaries within both the local Health and Social Care community and across partner agencies. This infrastructure will also need to provide the ability, with appropriate security, to use and interact with functions that reside in disparate underlying systems, and be capable of creating, storing and sharing data related to patients and pathways of care.
In addition the solution must be capable of appropriate interaction with a personal health record or a patient portal which may be developed from the solution or in line with National developments as they arise.
A clinical portal will therefore support as a minimum the delivery of the following:
– Improve the quality of patient care
– Improve service efficiency
– Support service transformation
– Improve key relationships
