This service will improve the outcomes for patient’s diagnosed cardiac conditions and their carers/families, by delivering evidence based interventions compliant with best practice and all relevant guidance/accreditation.
The service will ensure the co-ordination of an integrated pathway across primary, community, secondary care and social care, working in partnership with GPs, Cardiology Consultants, Specialist Nurses and Physiotherapists, Community Nursing and other Health and Social Care Professionals, in order to deliver a seamless pathway of care.
The estimated value of this contract is between a minimum of 2.4 million GBP and a maximum of 2.8 million GBP over a 3 year period.
The Contract will be for a term of 3 years with the opportunity to extend for two further years subject to mutual agreement between the Provider and the Commissioner. The maximum term of contract is consequently 5 years.
