Both CCGs have clear strategic priorities to improve both access to and the quality of care for their populations, providing care closer to home and contributing to the development of an efficient and financially sustainable health economy. Effective management of people with diabetes will help both CCGs improve outcomes for people with diabetes across all domains of the NHS Outcomes Framework.
NHS Brighton and Hove CCG and NHS High Weald Lewes Havens CCG are therefore seeking to establish a Community Diabetes Hub (and Spoke) model that supports the provision of integrated care for patients with a diagnosis of diabetes and the prevention of diabetes for those at risk, equitably across both CCGs. Both CCGs are looking for innovative solutions that will meet the needs of a growing population of people living with diabetes. For the purposes of this advert the service will be referred to as the ‘Hub’.
The Hub aims to improve health outcomes for people at risk of diabetes or who have a diagnosis of diabetes. The Hub will have a co-ordinating role, bringing stakeholders from primary care, secondary care, public health, and the community and voluntary sector to drive up the quality of care and support for people living with diabetes. The Hub will build the skills, capacity and expertise in primary care to increase the proportion of the diabetic population who are in control of their condition or are being proactively managed within primary care. The Hub will also work closely with secondary care to ensure patients are seen in the most appropriate care setting according to their clinical needs.
The Hub will be a specialist multi-disciplinary team (MDT) that will include consultant medics, nursing, podiatry, psychological support, dietetics, exercise and structured education, and will utilise a range of consumer mobile and health technologies to support self-management. It will have a role in providing specialist advice/education to support the management of patients within primary care. Hub staff will support joint care planning within primary care, empowering patients to manage their condition, they will provide direct clinical care for complex patients, and provide early proactive care for those at risk of diabetes related complications. The Hub will have strong links and pathways into and out of a range of ancillary services such as orthotics, district nursing and other community services.
The primary contract period will be 3 years from commencement of the service. The Authority may extend the contract for up to a further 2 years. The value range stated is based on the primary period of the contract (3 years).
This is a voluntary notice, as these are ‘Part B’ services. Although the CCG is following a procedure which is akin to the restricted procedure, the use of a voluntary Contract Notice and of terminology which reflects the restricted procedure does not indicate that the CCG accepts that it is bound by the provisions of the Public Contracts Regulations 2006 (as amended) and/or the Public Procurement Directive (2004/18/EC) save for the provisions of the Regulations and the Directive which apply to Part B services.
