Community health services. Health services. Tenders are invited by Stoke-on-Trent City Council to seek a qualified provider to manage and develop the provision of a Health Trainer based Lifestyle Service for the targeted populations that the Lifestyle Programme serves in Stoke on Trent. The Lifestyle Service will provide dedicated support, advice, education and motivation for up to 3 000 people a year who enter the Lifestyle Programme. The Lifestyle Programme has the key aim of helping people who are overweight and at high risk of CVD to achieve positive behaviour changes that also translate into measurable physiological changes ie weight reduction, smoking cessation, reduced alcohol intake, improved diet, increased physical activity, etc, within 6 to 12 months of entering a programme of support.
The contract will be available for other Local Authorites, Public Health and NHS Organisations to join during the contract period.
Please note that this is just for information purposes and there is no guarantee that any other Local Authorites, Public Health and NHS Organisations will opt to utilise the contract.. However, an approximate extra value for other Local Authorities, Public Health and NHS Organisations of 1 000 000 GBP is included in this contract notice.

Information and formalities necessary for evaluating if the requirements are met: The Authority will apply all the offences listed in Article 45(1) of Directive 2004/18/EC (implemented as Regulation 23(1) of the Public Contract Regulations (PCR) 2006 in the UK) and all of the professional misconducts listed at Article 45(2) of Directive 2004/18/EC (see also Regulation 23(2) in the PCR 2006) to the decision of whether a Candidate is eligible to be invited to tender.
