There are 2 distinct elements to the service – Domiciliary Care between the hours of 7:00 am and 11:00 pm 365 days per year.
Carer breaks to be available 24 x 7, 365 days per year.

St Helens Council is seeking to award a contract for the delivery of a Block End of Life Domiciliary Care Service within St Helens.
There are 2 distinct elements to the service – Domiciliary Care between the hours of 7:00 am and 11:00 pm 365 days per year.
Carer breaks to be available 24 x 7, 365 days per year.
This service will cover the whole of St Helens and be available to:
— people over the age of 18 and resident in St Helens who have a St Helens GP and with a life limiting condition with a 12 week end of life diagnosis (confirmed by a medical professional),
— carers supporting their family member at home and the family member meets the above criteria.
The provider of this service will receive all referrals and is required to commence delivery of care or a break within 24 hours of receipt of a referral.
Contract will run for an initial period from 14.1.2019 to 31.5.2021, subject to an annual review, with an option of 2 12-month extension periods.
