Tender for the Provision of a Dementia Care Advisor Service
The Council are seeking a suitably qualified and experienced organisation to provide a Dementia Care Advisor Service.
The Dementia Care Advisor Service will support the delivery of the Buckinghamshire Joint Commissioning Strategy for Dementia. The strategy takes into account feedback from people living with dementia and their carers. In particular, how the Council could improve the delivery of services and support in Buckinghamshire. The feedback indicated that people with dementia and their carers wanted choice and control over how they live their lives.
Buckinghamshire Dementia Care Advisor Service provides a mechanism for facilitating access to appropriate care, support, information and advice, from beginning to end of the dementia care pathway. The information will focus on the services and support available to people at the point of diagnosis and throughout the course of their care.
Provision of the Dementia Care Advisor Service will include: ensuring that the service is accessible for any person with a diagnosis of dementia, focus on well being rather than illness, support people to think through how they can come to terms with and live well with dementia and act as a conduit for information to facilitate access to and understanding of local health and social care support available. The service will also be available to people with a suspected dementia and will signpost them to and support them through the diagnostic process.  The service will have a team of volunteers under the supervision of The Dementia Care Advisor.
