Provision of Domiciliary Care Services for CHC funded individuals in the community, as required by the Isle of Wight NHS CCG Funded Care Team The requirements of Domiciliary Care Service provision to CHC funded individuals. Objectives are: To provide CHC funded individuals with access to safe, high quality, accredited Domiciliary Care Services; To deliver affordable and Value for Money (VfM) Domiciliary Care Services; To provide services that comply with accepted best practice, relevant guidelines in the delivery of care and robust governance arrangements; To provide a complete holistic person centred focussed care package; and To improve individual’s choice of provider. To enable individual’s to remain in their own homes and in control of their daily lives. This will be under the Any Qualified Provider (AQP) framework, and no guarantees of activity will be provided, this process is the first window of an existing AQP framework. AQP documents can be accesses and loaded through the In Tend portal;
