End of Life Carer Support Service Nottinghamshire

The aim of this service is to provide high quality respite with practical, emotional, spiritual and social support to the carers of people deemed to be within the last year of life. This will enable the carer to provide care for as long as they wish, and for both carer and cared for to plan and to empower the person cared for to choose the place in which they wish to die.
The provider will work within the community setting, in partnership with other services. They will arrange short breaks to provide respite for carers and some post bereavement support. The provider will offer reassurance, guidance and basic training in personal care tasks that the carer wishes to provide and support them in understanding clinical interventions.
The service must provide at least one member of staff with evidenced skills in training others to deliver personal care and all staff must have end of life specific training.
