Service Description
People approaching the end of life and their carers should have rapid and direct access to the assessments, interventions, care, advice and support that they require.
The Night Sitting service encompasses three key elements of provision:
Assessment, Treatment and Management;
Coordination of Care (in the last few days and hours of life); and
Training, Education and Quality Improvement.
The service delivers support to high quality End of Life Care and has a key role in enabling people to die in their preferred place of death where this is in “out of hospital” settings, working in partnership with health, social care and voluntary sector services and organisations in order to ensure integration and a continuum of care.
Education and training opportunities as well as advice and support are provided to support local health and social care staff, including professionals working in GP practices, care homes (nursing and residential), domiciliary care and community services in order to facilitate improvement in End of Life Care.
