Evalution of Link Workers in General Practice

NHS Tayside is seeking to commission an evaluation project on behalf of the Scottish Government to explore the effectiveness and practicability of Link Workers in specified General Practices in Dundee.

Background

GPs in socio-economic deprived areas experience a high volume of patients who attend with a multitude of issues that are not all necessary clinical. The Link Worker approach will have a focus on supporting patients to access non-clinical sources of support as a route to improving mental wellbeing and reducing frequent attendance in primary care. The Scottish Government project will complement the wider work that is currently being taken forward around social prescribing and mental health improvement in Dundee.

The Project

The link workers will support GPs in the two Deep End practices in Dundee by easing difficulties in managing patients who present with socio-economic concerns. GPs will identify and refer appropriate patients to link workers, who will encourage the uptake of a wide range of services and activities including, where appropriate, physical activity opportunities.

Evaluation

The evaluation will take place between August 2013–15. The successful provider will be expected to advise on the development of the most appropriate model taking into account the experience of the existing scheme in Dundee and the wider evidence base for social prescribing. The evaluation will include a component related to the on-going experience of the existing scheme in two non-Deep End practices.

The evaluation will focus three aspects:

1. The Development phase: refining the project: roles, structures and partnership working

2. The Operational phase: bedding in the link worker; referral systems; working with patients; accessing local resources

3. Outcomes phase: at patient; practice; and local resource level.
