Description of the contract

Central London, Hammersmith and Fulham and West London Clinical Commissioning Groups (CCGs) wish to engage with suitably qualified service provider(s) for the provision of Expert Patient Programme, Diabetes Champions and Diabetes User Group within the boundaries of the CCGs (London boroughs of Hammersmith & Fulham, Kensington & Chelsea and Westminster). This tender opportunity is being procured via the SBS e-tendering portal through this link https://uk.eu-supply.com/app/rfq/viewtransaction.asp?TID=18371 The Expert Patient Programme (EPP) is a structured 6-week course, delivered by Lay Tutors living with a Long Term Condition, in accordance with the Chronic Disease Self-Management Programme developed by Stanford University. The aim of the Expert Patient Programme (EPP) is to enable people with long-term conditions to: • Feel confident and in control of their lives • Aim to manage their condition and its treatment in partnership with health care professionals, e.g. GPs, Practice Nurses, Consultants • Communicate effectively with professionals and be willing to share responsibility of treatment • Be realistic about the impact of their condition on themselves and their family • Use their skills and knowledge to lead full lives The service is to be delivered within the Central London, Hammersmith & Fulham and West London CCG boundaries. The Diabetes Champions is a volunteer-led initiative to target local people with, or who are at risk of, developing Type 2 diabetes. The aim of the service is to help raise awareness of Type 2 diabetes and risk factors, and help people make better lifestyle choices. The Diabetes User Group provides a targeted forum to ensure that the voices of the local population contribute directly to the provision and development of local diabetes services. All three services will form one contract and while each has specific outcomes, they all utilise the experience and passion of local people to improve the health of the broader population, specifically those with long term conditions. The term of this contract will be for 3 years with the option to extend for a further two 1 year periods (3+1+1). The payment schedule for the Expert Patient Programme will be part block and part activity, with an estimated 600 participants in year one. The payment schedule for the Diabetes Champions and Diabetes User Group will both be block. Bidders are advised that TUPE will be applicable to this contract. This service will be delivered in accordance with the NHS Standard Terms and Conditions.
