The intended outcomes of the Home Care Support Services will be clearly identified on each child or young person’s Care Plan. They could include helping the child or young person:
— To provide practical support to parent carers.
— To support restoration back into the family network.
— To maintain the current levels of physical, mental and social well-being where improvement is not envisaged.
— To contribute to the emotional and social well being of the child or young person.

To provide support within the child/young person’s own home to support the parent/carer with the personal/physical care of the child/young person in those situations where the parent/carer is not able to deliver it on their own. This may include associated tasks with managing daily life.
