Supports services for single homeless will be tendered in two lots:
Lot 1 — Homeless Support Services Hub Centre and Outreach
— The development and provision of a referral and assessment hub centre capable of providing multi agency services and interventions
— The service will provide short term interventions, advice and support both to enable homeless households to quickly find and establish themselves in their own accommodation and to prevent homelessness for vulnerable tenants faced with eviction
— A triage system will operate with access to long term support for people with more complex and long terms needs
— A key requirement of the service is the provision of more highly qualified support navigators who are able to offer effective housing and welfare rights advice as well as personalised outcome driven support, which will be a key feature of the performance framework linked to Payment by Result (PbR).
— The Outcome Performance and PbR Framework to be designed by the potential provider.
Lot 2 — Support Services to Homeless Young People.
— To provide an accommodation based support. The service will also be expected to provide an outreach service to young people who move on from the supported units or who are already living in their own accommodation but still need support.
— Support navigators that are flexible, passionate, highly experienced and qualified to offer consistent and effective information and advice across housing, welfare rights etc.
— Support navigators that are able to offer effective mentoring; who facilitate peer support but who also enable independence
— Small scale and safe accommodation based services with consistent on-site support that are homely but which enable young people to have their own space
— Support navigators demonstrating delivery against personal outcomes, improving well being
— Support pathways that support young people into their own homes and which are clear to young people
— Multi-disciplinary responses and skills aimed at people with more complex needs
— The Outcome Performance and PbR Framework to be designed by the potential provider.
Providers may wish to apply for 1 or both lots. The Contract will be for a 3-year period with the option to extend by a further year, subject to annual review.
