Description
Housing related support services for people with a mental health diagnosis within a variety of different properties and schemes across West Berkshire. This contract is initially for 3 years with the option to extend for an additional 2 years.
The provider will work with the service users to:
Maintain tenancies.
? Provide person-centred support, which takes full account of diverse needs and lifestyle choice.
? Promote Client’s choice, well-being and safety.
? Promote people’s independence by focusing on abilities and encouraging people to be as self-directing as possible.
? Manage the home environment.
? Manage finances, maximise income/benefits.
? Encourage access to employment/education.
? Encourage meaningful day-time activities, community access and integration.
? Engage with relevant professionals, i.e., Mental Health Team, Housing Options Officers, Employment agencies
? Establish and sustain effective social support.
? Reduce health inequalities by maximising access to income, learning and work.
? Advise on personal care.
? Provide high quality, cost effective and efficient services.
? Promote equality of opportunity and anti-discriminatory practice
