Description of the contract

Intermediate Care Services – Call for Expressions of Interest (EOI) Torbay and Southern Devon Health and Care NHS Trust (the Trust) wishes to hear from organisations interested in providing Intermediate Care Services in residential and nursing home settings. The Trust is the only integrated community health and adult social care organisation in the South West and provides Intermediate Care to the area covered by Torbay and Southern Devon. Intermediate Care is a range of integrated services provided for a limited period of time to promote faster recovery from illness, prevent unnecessary acute hospital admission and premature admission to long-term residential care, support timely discharge from hospital and maximise independent living. The emphasis of Intermediate Care should be on active enablement. Intermediate Care services are free of charge to service users. The outcome and services required for Intermediate Care are: • To provide time limited nursing and residential home placements for clients who have a short term acute medical problem that prevents them being cared for safely at home but where the responsible doctor does not deem hospital admission necessary. Multidisciplinary assessment and rehabilitation are provided by zone/cluster based Intermediate Care teams. • To reduce the number of people prematurely or inappropriately being admitted or re-admitted to hospital or into a long stay placement within a residential or nursing home setting. • To support earlier hospital discharge for those patients medically fit enough to be cared for in the community from both Acute and Community Hospitals. • To increase the scope for Intermediate Care placements and ensure that all clients can receive Intermediate Care within a care home setting. An example of this is to increase the availability of placements for younger clients and for those who may have challenging behaviour. • The Trust would like to explore the possibility of Intermediate Care being provided by a Care Home in the patient’s home setting. This may take the form of providing input to a patient once they have been discharged home from placement, or provision in place of a Care Home placement. Providers are encouraged to explore this opportunity within their tender and outline what additional services they may be able to provide that could meet this need.
