To deliver the carer’s strategy priorities the Council is looking for a lead provider who will assume the accountability for delivery of all support for carers in Knowsley. The support for carers will focus on maintaining or improving the health and wellbeing of carers and helping to sustain caring relationships where appropriate and possible. The contractual relationship will focus on relevant outcomes for carers and the cared for e.g. preventing residential and nursing care admissions. Support will be prioritised for those caring relationships that are identified as high risk of failing, with rapid access to appropriate support to stabilise the situation, reduce the risk, and maintain and improve the health and wellbeing of the carer and the cared for.
