Pass/Fail Criteria.
Financial (Pass/Fail).
To ensure the successful Contractor has the capacity to deliver this Contract Midland Heart requires turnover to be 2x the value of the Lot/Lots applied for per annum.
Please submit a copy of your accounts for the past 2 years which shows your turnover as evidence that you can meet this pass/fail criteria.
Insurance (Pass/Fail).
Midland Heart requires the following insurance levels for this Contract:
— Professional Indemnity Insurance 1 000 000 GBP,
— Public Liability Insurance 5 000 000 GBP,
— Employees Liability Insurance 5 000 000 GBP.
Please submit a copy of your insurance certificates demonstrating you meet this criteria, or alternatively submit a statement that you will put these levels of insurance in place if you are successful in being awarded this Contract.
Probity Statement (Pass/Fail).
Please sign the Health & Safety Code of Conduct and the Probity Statement in Appendix 19 and 20 and submit with your tender response.
