NHS Health Checks in Buckinghamshire – Prior Indication NHS Health Checks in Buckinghamshire – Prior Indication Notice (PIN) Buckinghamshire Primary Care Trust is seeking informal expressions of interest from organisations able to provide the invite and screening components of the NHS Health Checks programme. NHS Health Checks will be offered to individuals aged 40-74 every 5 years. Each check should include an assessment of cardiovascular (CV) risk, an explanation of CV risk to the patient, a face to face discussion about risk reduction, brief interventions where relevant, and onward referral or signposting to supportive or treatment services as necessary. The general aspects that may be commissioned are as follows: • Patient identification • Patient invite and follow up of non-responders • Screening appointment (QRISK2 calculation and face to face discussion on risk reduction) • Secure data transfer (read coded format) into patient records GP practice (if screening not taking place in GP practice) • Additional work to ensure under-represented groups attend for screening appointment Specialist risk management and lifestyle interventions, such as smoking cessation and weight management, will be provided through services already commissioned by the PCT; we are not seeking expressions of interest for these services. Depending on the capacity of forthcoming providers, a competitive tender process may follow. This would seek providers who could offer one or more components of the Health Checks pathway, with the potential for the whole pathway to be delivered by one or more providers. Provision would need to meet the minimum standards set out in relevant Department of Health guidance and other relevant standards and guidance. The eligible population to be invited for the NHS Health Check in Buckinghamshire is estimated to be around 36,000 each year. It is possible this provider will be responsible invite and screening up to 18,000 people each year. This number will be finalised In January 2012, following performance review of the primary care Locally Enhanced Service (LES).
