Islington Council, Islington Primary Care Trust (as novated) and associated partners invites suitable expressions of interest from providers who specialised in the provision of non exclusive residential, nursing, social care and other related services for Older Adults and/or Adults with a Learning Disability and/or Physical Disability in Registered Care Homes within Greater London, Kent, Surrey, Berkshire, Buckinghamshire, Bedfordshire, Hertfordshire and Essex.
This framework agreement has been divided into two lots:

    • Lot 1 – Older Adults
    • Lot 2 – Adults with a Learning Disability and/or Physical Disability

The service objectives are:
·      To promote the independence of adults assessed as needing social care support by placing them central to decisions that have an impact on their lives.
·      To enable choice and the opportunity to exercise choice through the provision of full information about the service and range of facilities.
·      To ensure the protection of the Service Users, safeguard and promote the health, welfare and quality of life for the Service Users.
·      To promote close working with the local health trusts (primary care, acute and community), carers and other relevant agencies to avoid unnecessary admission to hospital and inappropriate placement upon leaving hospital.
·      To enable service users, carers and families to take a full and active part in planning, monitoring and reviewing services.
·      To ensure care plans are reviewed as necessary so that they continue to be appropriate and effective.
·      For the Supplier to work in co-operation with the council to achieve good quality residential and nursing care provision minimum requirements.
