There is a requirement for an online database and the provision of fully equipped premises that are accessible to employees based at various locations across Staffordshire, West Midlands and Warwickshire. The contract will be for the provision of an Occupational Health Service to provide medical assessments, consultations, opinions and reports relating to the health and well-being of approximately 2 100 existing group of diverse employees as and when required. There is a requirement for an online database to support the provision of health information, tools, forms and information as well as being able to support the booking of appointments and the tracking of cases. The provision of fully equipped premises that are accessible to employees based at various locations across Staffordshire, West Midlands and Warwickshire. The contract will be for two years with an option to extend for a further 12 months, commencing on 1.4.2013. The closing date for receipt of tenders is at 14:00 hours on Wednesday, 4.102012.

Information and formalities necessary for evaluating if the requirements are met: The Authority will apply all the offences listed in Article 45(1) of Directive 2004/18/EC (implemented as Regulation 23(1) of the Public Contract Regulations (PCR) 2006 in the UK) and all of the professional misconducts listed at Article 45(2) of Directive 2004/18/EC (see also Regulation 23(2) in the PCR 2006) to the decision of whether a Candidate is eligible to be invited to tender.
