Portsmouth City Council wishes to let two contracts for Dementia support:
Lot 1 – The Patey Memory Centre is required to provide day centre opportunities for those with moderate to advanced dementia, promoting the retention of life skills and enabling those attending the service to live as well and safely as possible with dementia.
Lot 2 – To provide a Portsmouth Dementia Community Support Service which will co-ordinate the availability of information around dementia services to the local community.

1)Short description

To provide day service opportunities to people with dementia both within a centre environment and within the community. To promote the retention of life skills and enabling those attending the service to live as well and safely as possible with dementia.

The Patey Memory Centre is required to provide opportunities for those with moderate to advanced dementia, promoting the retention of life skills and enabling those attending the service to live as well and safely as possible with dementia.
The service will strive to promote the current best pratice in dementia care and proactively improve the quality of life for their service users:
Assisting older people with dementia to achieve and sustain good mental and physical health and personal well being including including supporting people with dementia.
Maximising an individuals potential to live in their home amd maintaining social engagement through a personalised and person centred approach to care.
Empower and enable carers by supporting community based peer support for carers.
Inspire service users to contribute to live in their community and to be confident in living as full and active of a life as possible maximising their potential at home and within the community.

To provide a Portsmouth Dementia Community Support Service that will co-ordinate the availability of information around dementia services to the local community.

— provide a level of telephone and e-mail advice and support for those with dementia and their carers to signpost them to correct services across the city,
— to work with local colleagues to provide integrated services across statutory and voluntary organisation within the city,
— support the creation of a self supporting user lead support group.
