Imperial College Healthcare NHS Trust is seeking expressions of interest for a cost effective, efficient specialist administrative resource to pilot an innovative approach in supporting our patients, their families and carers in finding ongoing care if required on discharge from our hospitals.
This resource will bring market expertise and work closely alongside our teams when preparing patients for complex discharge and work collaboratively across the multi agency care system providing continuity and advocacy for our patients through this process.
This will require an expertise in this field with a proven track record of delivering these services, presence on site in our hospitals and 7 day a week point of contact.
Any successful supplier will need to work to clear and robust targets and Key Performance Indicators ensuring patients are supported in making informed decisions.
