PROVISION OF A PEER INFORMATION, ADVICE AND SUPPORT SERVICE TO PROMOTE, AND ENABLE INDIVIDUALS TO ACCESS AND MANAGE, A DIRECT PAYMENT The council is committed to supporting people to achieve better health and social care outcomes in their lives. We are transforming our delivery of health and social care through the implementation of personalisation (self-directed support). Council policy is that the preferred method of delivering social care is through a Personal Budget, taken as a Direct Payment. We are seeking to appoint a Provider of a quality information, advice and support service with the prime focus of increasing awareness of Direct Payments and encouraging / enabling take-up by people who are eligible. The service will be delivered by peers and will support individuals to make the most of their personal skills, experiences, connections and interests in managing their Direct Payment, as well as creatively accessing community assets and resources. Organisations will be asked to detail the inputs, processes and outputs they believe will best deliver against our outcomes-based Specification – a draft of which is attached with this advert. There will be a “payment by results” element in this Contract award. This Pre Qualification Questionnaire stage will close at 2.00pm on Friday 11th October 2013. The contract for this service will be for a period of 2 years with an optional 2 year extension period.
