The Authority is seeking to commission a Rehabilitation Service as a support service for customers who are newly diagnosed with visual impairment/dual sensory loss, those who have lived their life with reduced sight loss and those who are deaf-blind. The service will also meet the needs of those already in receipt of a Certificate of Visual Impairment who experience a change in their vision.
To prevent customers from experiencing crisis that would require ongoing social care services the provider will promote the benefits of registering their sight loss to potential customers and the Rehabilitation Services available to them. This will require the Provider to work in collaboration with local hospital services and Eye Clinics to raise awareness of the support available across the County.

Our aim is for the service to provide support and rehabilitation services to visually impaired adults aged 18 years and over living in Warwickshire.
The successful applicant will promote the Rehabilitation Service as a support service for customers who are newly diagnosed with visual impairment/dual sensory loss, those who have lived their life with reduced sight loss and those who are deaf-blind. The service will also meet the needs of those already in receipt of a Certificate of Visual Impairment who experience a change in their vision. The successful applicant will also prevent customers from experiencing crisis that would require ongoing social care services the provider will promote the benefits of registering their sight loss to potential customers and the Rehabilitation Services available to them.

The contract start date is 1.10.2016 with end date of 30.9.2019. The option to extend the contract by further 2 years will be at the discretion of the Authority based on the contract performance. The service may be developed and modified during the course of the contract period to meet the changing demands, requirements and needs of the customers or the Authority.

The Council will undertake its financial assessment of the tendering organisation (and if considered necessary by the Council any partners within a consortium bid) based on its own evaluation of the tenderers most recent 3 years accounts. If 3 years accounts are not available equivalent evidence which confirms financial viability. The assessment of accounts will be supported by an independent credit reference report from Creditsafe where this is available. The Council may also impose a bond or performance guarantee on the successful Tenderer.
