Kent County Council is embarking on a journey to transform social care in Kent. Within the context of this opportunity, it is imperative that individuals are at the heart of the care and support they receive, which should be easy to access, of good quality and that maximise their ability to live independently and safely in their community.

This commissioning exercise will include residential care home services (learning disability, physical disability and mental health needs), supported living services, discharge support services and the community lunchtime service. This process will also include the opportunity to bid for care and support in the home services (Homecare and Community-Based SiS) across limited areas of Kent.

Lot 1 (LD, MH and PD Residential Care Home Services — Specialist and Specialist Plus) includes those individuals aged 18+ who have been assessed as requiring a residential care service, whose primary assessed need is in relation to a learning disability, physical disability or mental health and whose need level has been assessed as specialist. Objective criteria for choosing the limited number of candidates:

The Council has estimated that up to 200 providers may be needed to join this framework, based on current demand. Tenderers that have met all mandatory criteria, and have submitted pricing in line with the cost model document will be considered for a place on the framework. Consideration must be made to having a spread of potential residential providers across all geographical areas of Kent and across the required needs categories. Lot 4 (Discharge Support Services) incorporates the discharge to assess, home to decide and home to settle services. The aim of the discharge to assess service is to provide ‘wrap around’ support to people in their own homes for up to 3 days post discharge from hospital (with potential to extend for a further 2 days if the person’s outcomes will either negate the requirement for ongoing service or reduce the need). The Home to Decide Service provides a person with an intensive 72 hour nurse led assessment period following an acute episode of care in hospital, the opportunity to have a holistic health and social care assessment in their own home environment and provides an analysis of both physical and emotional improvement out of an acute setting. The aim of the assisted discharge (Kent) service is to provide support for up to 3 days initially, supplemented by other support services from the statutory or voluntary sector such as advocacy, benefits checks and social support/activities where appropriate.
