Description
Nottingham City Council are seeking to continue providing a targeted and holistic service to rough sleepers and those at risk of rough sleeping to ensure that rough sleepers are resettled and enabled to live independently.
The aim is for this to be the primary service to meet Nottingham’s No Second Night Out objective. To make contact with and support those sleeping rough within the City of Nottingham, develop short-term action plans for each individual, engage them in meaningful activities, make referrals to the Homelessness Prevention Gateway and other services as appropriate and to undertake reconnection. To deliver outreach sessions within existing appropriate services to better enable contact with clients.
This service will be for rough sleepers and those at risk of rough sleeping who are either sleeping rough in Nottingham or have a local connection with Nottingham but are sleeping elsewhere.
