The objective of this project is to commission a Self-harm pathway care bundle for children and young people across NHS Nottingham City CCG, through the delivery of high quality healthcare services.
The key aims and objectives include the following key outcomes:
— Reduction in risk taking behaviour.
— Young people report improved self-esteem and emotional wellbeing.
— Reduced requirement for specialist mental health services.
— Increased participation in healthy activities, reduced isolation.
— Improved family functioning and relationships.
— Young people remain or re-engage in education and training.
— Reduced risk of anti-social behaviour and offending.

Description of particular conditions: The Contract/s to be entered into by the Commissioners and the selected Provider(s) for the Procurement will be based on the 2015/16 NHS Standard Contract (http://www.england.nhs.uk/nhs-standard-contract/) and will comply with the requirements of the Service. Each Contract will be separate to and independent of any existing contract currently in place between a Provider and the relevant Commissioners.
