Lot 1 — Outreach Support service with Supported Accommodation;
— To assist the Council to meet their statutory accommodation and support duties to homeless individuals/couples;
— To provide outreach support for up to 50 individuals in the community at any one time;
— To manage a 12 bedded Supported Accommodation facility for homeless adult households with complex / multiple needs in the East Kilbride area;
— To develop detailed support plans for individuals using the service;
— To ensure that all individual support plans and reviews are effectively co-ordinated;
— To develop positive working relationships with other services in order to provide effective integrated responses to vulnerable households;
— To make pro-active attempts to retain contact with and offer continued support to service users;
— To report outcomes on a regular basis in order to evidence the success of the service;
— To work flexibly with the Council to meet the support and accommodation needs of individuals with complex needs.
The Supported Accommodation will be operational 24 hours per day / 7 days per week and will be staffed to provide a service at full capacity at all times. The service will also provide 24/7 on call support to Service Users receiving Outreach Support.
Lot 2 — Outreach Housing Support for Households with children;
— To provide a holistic, client focused floating Housing Support Service for 30 families (at any one time), who have complex needs and who are homeless or at risk of homelessness;
— To develop detailed support plans for individuals using the service;
— To ensure that all individual support plans and reviews are effectively co-ordinated;
— To develop positive working relationships with other services in order to provide effective integrated responses to vulnerable households;
— To make pro-active attempts to retain contact with and offer continued support to service users;
— To report outcomes on a regular basis in order to evidence the success of the service;
— To work flexibly with the Council to meet the support and accommodation needs of individuals with complex needs.
The service will be available between 8.45 am and 4.45 pm Monday to Friday, however, the Provider will ensure that a system is in place whereby they can provide a Housing Support Service to households outside these hours where this is considered necessary. The service will operate from a Provider sourced office in South Lanarkshire.

It is anticipated that the total contract value will be approximately 1 280 000 GBP over the two year period duration split as follows:-
— Lot 1 — 960 000 GBP;
— Lot 2 — 320 000 GBP.

Description of particular conditions: 1 — to be registered with the Care Inspectorate or equivalent to deliver Housing Support services.

(2) All candidates will be required to produce a certificate or declaration demonstrating that the candidate, their directors, or any other person who has powers of representation, decision or control of the candidate has not been convicted of conspiracy, corruption, bribery, or money laundering. Failure to provide such a declaration will result in the candidate being declared ineligible and they will not be selected to participate in this procurement process.
(3) All candidates will be required to produce a certificate or declaration demonstrating that they have not been convicted of a criminal offence relating to the conduct of their business or profession.
(4) All candidates will be required to produce a certificate or declaration demonstrating that they have not committed an act of grave misconduct in the course of their business or profession.
(5) All candidates must comply with the requirements of the State in which they are established, regarding registration on the professional or trade register.
(6) Any candidate found to be guilty of serious misrepresentation in providing any information required, may be declared ineligible and not selected to continue with this procurement process.
(7) All candidates will have to demonstrate that they are licensed, or a member of the relevant organisation, in the State where they are established, when the law of that State prohibits the provision of the services, described in this notice, by a person who is not so licensed or who is not a member of the relevant organisation.
