Description of the contract

The overall aim of the service is to provide a person-centred supported living service for adults with profound and multiple learning disabilities delivered within the framework of an individual service fund approach. This individual service fund framework aims to extend personalisation and choice in shared supported housing schemes, in particular for people who have complex care and health needs. Central to the model will be service user choice, independence, community participation, financial accountabilty and partnerships with families, health and other agencies. Commitment to leadership and providing an experienced, qualified, stable workforce who are motivated to achieve the best for service users is essential. As this is a new service, commitment to supporting the transition and seamless start of this service is essential.
