- St.Helens Council Public Health Department is looking for supplier who can provide either (or both) of the following requirements.
Requirements are provided as individual lots.
LOT 1: Requirement for a provider to support data extraction from general practice to meet the needs of the Public Health Contract in St. Helens Merseyside
We are a local authority in the North West of England with 37 general practices with whom we commission the following services:
· NHS Health Checks (based on national outcomes data)
· Intra –uterine contraceptive devices
· Contraceptive Implants
· Chlamydia Screening in the 15 – 24 year old population
· Shared Care (Drugs)
We wish to commission an organisation that can extract data from general practice systems to meet the needs of the service specifications both payment and outcomes data. The local authority do not require patient level data but will require data that meets the standards in relation to payments for practices and provides a level of information about patient care and outcomes. The organisation will need to work closely with all the general practices to ensure that this is implemented.
LOT 2: Requirement for an IT system to support the NHS Health Checks Programme in St. Helens
St Helens Local Authority has reviewed their existing Health Check programme which is currently provided solely within general practice and as a result the programme will be expanding and a new community provider will be delivering NHS Health Checks.
We are looking for an IT system that can be used in general practice as a front end to the clinical system, community and outreach setting to support the NHS Health Check programme in St. Helens. The system should be able to support the providers to deliver the NHS Health Check and have the necessary tools embedded to calculate scores such as GPPAC, CVD risk score, BMI, based on national requirements.
The system should support the person undertaking the Health Check to provide the patient with an individual care plan at the end of the Health Check. For the community provider the system should be able to produce data and reports should that support the commissioner in relation to activity and the outcomes of patients and the effectiveness of the programme. The system should also be able to provide the data back to general practices for their responsible patient and be able to link with their clinical systems which are: Vision and EMIS Web.
