This contract is for the Falls Prevention Community Exercise Programme which is being commissioned by the Council’s Adults, Wellbeing and Health directorate.
The Falls Prevention Community Exercise Programme will provide a mix of 10 weekly Postural Stability and progressive Chair Based Exercise Groups within designated localities within County Durham.
The overall purpose of the service will be to promote bone health, improve the strength, balance, confidence and independence of older people primarily aged 65 and over who are either at risk of falling or who have suffered one or more falls with a view to:
o   Generally improving older people’s strength and balance;
o   Improving older people’s ability to get up off the floor if they do fall, thus preventing a ‘long lie’*;
o   Maintaining older people’s independence for longer;
o   Reducing the likelihood of future falls;
o   Reducing the costs to both health and social care incurred as a consequence of falls related injuries.
 
The focus of the service will be on those people at risk of initial or future falls (primary prevention) and those who have suffered repeated falls or who have sustained significant injury from a fall (secondary prevention/rehabilitation).
 
This service will provide a consistent and equitable approach across the County.
