Worcestershire County Council has funded a Home from Hospital Service since April 2000. This service is part of a range of preventative services aimed at supporting people in their own homes and to assist them in improving and/or retaining their independence. This service develops the existing hospital at home service into a wider support at home offer for general support for independence, that combines short term practical support to enable people to remain independent at home while linking the individual with existing support structures and community health assets unique to their own situation. The current arrangements for the provision of these services are due to end on the 31.10.2019 the Council working in partnership with Worcestershire Clinical Commissioning Groups, wish to procure a countywide support at home service from the 1.11.2019.
